The ICDC program assists people with chronic disease to access allied health services and manage their disease.
See your GP for a referral to the ICDC program and have a current GP Management Plan and Team Care Arrangement.
This service is supported by funding from WA Primary Health Alliance under the Australian Government’s PHN Program.
Wheatbelt Health Network (the Network) is a not-for-profit organisation that provides multidisciplinary primary care services across the Wheatbelt.
Services include GP, nursing and allied health. The operations are overseen by an experienced Board and implemented by a talented and progressive management team. The Network is a key regional player in delivering health care services with the super clinic in Northam being the organisation’s headquarters.
